APPOINTMENTS

Patients are seen by appointment only. Routine appointments can be made by calling our office at (724) 547-4575 or (724) 832-9688.

We will TRY OUR VERY BEST to accommodate your schedule. Urgent appointments are scheduled as soon as possible. Voice messages and emails answered daily before the office hours and at the end of the business day.

Things to Remember When Visiting Our Office.

  1. Referral -Before you arrive, make sure your referral, if needed, is current.
  2. Insurance Card(s) — Bring your insurance cards(s) to every visit. We copy them and use the information to bill your insurance company directly. Insurance plans are frequently updated.
  3. Co-Payment — Bring your co-payment, if your insurance company requires one.
  4. Adult patients are required to bring photo ID at each visit.
  5. If it is your FIRST appointment, PLEASE ARRIVE AT THE OFFICE 20 MINUTES BEFORE YOUR SCHEDULED APPOINTMENT TIME TO COMPLETE YOUR CHART. Bring the completed forms which can be downloaded from our website. This saves time — both yours and ours. Call our office to find out which forms you will need. It is essential that you bring an accurate list of all medications and their dosages. If possible, please furnish copies of any pertinent x-rays and a physician's report for your first visit.

To Cancel an Appointment

We strongly urge you to call our office at least 24 hours in advance if you cannot keep an appointment. Everyone will appreciate your courtesy, and another patient will have the opportunity for an appointment. This will also ensure your medical care is continued appropriately.

Payment

We require payment, including co-payments, at the time of services, unless other arrangements have been made in advance. We accept Visa, MasterCard and American Express along with cash and checks. If you have no insurance, you are expected to pay the fees in full at the time of your visit.
